The "CreateBookmarksFromGroupTree" setting for PDFs really needs to be an RRD-level setting. You can set it in Formats.xml, but it's the kind of setting that simply doesn't apply across the board. A two-page report with a group structure that will only make sense to the developer certainly doesn't need bookmarks. But a 500-page downtime report showing provider schedules and patient profiles absolutely needs them. If I turn it on to accommodate that one report, all these other minor reports are going to gain this confusing new feature that they don't need.
